Getting Started

Getting started at AMFW depends on the type of care you’re seeking. The pathways below are designed to help make the process as smooth as possible.

Non-pregnancy Care

For women’s health, men’s health, hormone therapy, and other non-pregnancy services, getting started is simple.

Please contact our office to schedule an appointment or to ask questions about which type of visit may be most appropriate.

Pregnancy Care

Pregnancy care starts with our stepped intake process.

Step 1: Complete the Pregnancy Intake Form
This form helps us understand your health history, pregnancy details, and care goals.
If you are transferring care, you must also send a copy of your prenatal care records from outside providers. (Records may be sent by fax or email)

A member of our intake team will review your information and reach out by phone shortly.

Once we confirm:
1) our care is a good fit for your pregnancy, and
2) there are enrollment spots available for the month you are due,
you can proceed to step 2.

Step 2: Complete the Billing Information Form
This allows our billing team to prepare an individualized estimate of costs and insurance coverage.

Our billing partner, GM Billing & Recovery Solutions, will reach out with your estimate.

Step 3: Enrollment Acceptance
After completing steps 1 & 2, our intake team will contact you to discuss enrollment and next steps.
