Getting started at AMFW depends on the type of care you’re seeking. The pathways below are designed to help make the process as smooth as possible.

Non-pregnancy Care

For women’s health, men’s health, hormone therapy, and other non-pregnancy services, please contact our office to schedule an appointment.

Pregnancy Care

Pregnancy care starts with our intake process.

Step 1: Complete the Pregnancy Intake and Billing Information Forms
If you are transferring care, you must also send a copy of your prenatal care records from outside providers. (Records may be sent by fax or email)

Step 2: Confirm Eligibility and Review Cost Estimate
After completing the Pregnancy Intake and Billing Information Forms:

A member of our team reviews your information to confirm our care is a good fit for your pregnancy, and that there are enrollment spots available for the month you are due.
You will receive an email with your custom cost estimate from our billing partner, GM Billing & Recovery Solutions. If you do not see this email, please check your spam.

If you'd like to see our space, visit us during an Open House every Friday from 11:30am-12:30pm

Step 3: Complete Enrollment
After we have confirmed eligibility and you have approved your cost estimate, contact our office to complete enrollment and secure your spot for your due month.
