Patient Records & Consumer Complaints

You have the right to access your health records in accordance with applicable state and federal law. Current or former patients may access their records through our EHR (ClientCare) Portal. Requests for records transmission are handled through our office to ensure privacy, accuracy, and timely processing.

To request copies of your health records, please complete the appropriate records request form. Forms may be completed in person, electronically through patient portal, or transmitted by fax (214-495-9918) or email (info@allenmidwifery.com). Requests may require verification of identity and authorization prior to release.
