A Long-term Healthcare Home

At AMFW, we think of care as something that grows and shifts alongside people over time. Many people first come to us during pregnancy, then continue returning through different seasons of life as their needs change.

Our hope is to offer a trusted place for care that feels familiar. A place where people feel known, conversations are unhurried, and care is shaped with attention and respect rather than routine.

By practicing in partnership and approaching care with openness and trust, we strive to be a healthcare home people come back to with confidence, knowing they will be met with exceptional, individualized care as life unfolds.

A Midwifery-led Approach

A midwifery-led model of care centers clinical judgment, continuity, and respect for physiologic processes. This model differs from more intervention-forward approaches by emphasizing discernment over default escalation. It values knowing when to step in, and just as importantly, when not to.

Midwives are experts in supporting healthy pregnancy and birth as normal life events, while recognizing when care needs change and additional support is appropriate. This approach carries through all of our work, shaping care across different stages of life and health needs.

Our practice is informed by evidence and supported by established clinical guidelines, but not driven by protocols alone. Rather than treating care as a checklist, we draw on experience, clinical judgment, and an understanding of each person’s health needs, priorities, and goals to guide care. This allows us to individualize support, balancing watchful restraint with readiness to intervene when circumstances change.

Exceptional Care, Built on Partnership

Shared decision-making is central to how we practice, shaping conversations that are informed, collaborative, and grounded in clear clinical guidance. We take time to explain the reasoning behind our recommendations, discuss benefits and risks, and answer questions along the way.


At the same time, this does not mean every option is appropriate in every circumstance. Our responsibility is to offer guidance within context of the full clinical picture, helping each person weigh information realistically so decisions are made collaboratively, with clarity and confidence.

Safety, Scope, and When Care Changes

Health and pregnancy are not static, and care sometimes need to evolve. When needs change, we approach those conversations with transparency and steady guidance. This may include consultation with other providers, physician co-management, or transition to a different level of care, always with the goal of supporting the best possible outcome.

We view collaboration as a strength. When additional expertise is needed, our role is to help navigate those transitions clearly and respectfully, maintaining communication and continuity whenever possible. In this way, care remains responsive, appropriate, and centered on the well-being and values of each person as needs evolve.
